Significantly, the suicide inquest files in 1980s and 1990s contained a disproportionate number of schizophrenia cases compared to the 1930s and 1940s. Under-counting during earlier decades due to poorly described cases is a strong possibility, but a contributing explanation is the rushed de-institutionalization of the 1980s and the resulting gaps in psychiatric care.190 The quantitative evidence is suggestive rather than conclusive; however, it is joined by the testimony of witnesses. Substance abuse with psychotic episodes is another possibility for a likely increase in suicides.

Waves of new pharmaceuticals during the late twentieth century brought hope to individuals with a complex illness, but medication introduced problems too. Side effects ranged from the annoying to the life-threatening; effective dosages and drug interactions were imperfectly understood. Refinements to dosages, frank discussions about risks and benefits, studies on the relative merits of placebos and specific medications, and removals from treatment programs of some drugs, were measures that denoted greater caution. Initial enthusiasm and optimism faded with experience. Patients living outside hospitals went off medication for assorted reasons: side effects; periods of improvement; a conviction that problems were not medical; a feeling that the medication provided no help.
